        Communication to ensure consistent, well-coordinated care for clients by communicating clearly and routinely with internal and external personnel involved in clients’ cases, including proctor parents, caseworkers and/or parole and probation officers.
        Working to create a roadmap of SMART goals that are long term and short term, working on BRS requirements, engaging in outside treatment services to outline what is included in the Parole and Probation agreements and working to deliver services to Offense Specific and At Risk youth.
        Coordinate and monitor the full spectrum of client treatment to ensure that the client’s physical, mental, and educational needs are being met.
        Organizational skills to manage the home room activities, client schedules, etc.
        Documentation and investing in clients’ ongoing success by documenting client needs, behaviors, and support plans. Help ensure that our documentation is on time and meets state requirements.
        Proctor Management - Maintain consistent contact with proctor parents and assist in building a productive and meaningful relationship with the client and proctor parent.
        As a Case Manager, I am also managing respite for both the youth and proctor parents as needed.
        Working on Skill development to model and support client development of social and emotional skills. Teach and practice independent living skills like financial literacy, time management, and communication.
        Consistently communicate with the education team to monitor progress towards graduation and problem solve issues with education staff with the client.
        Delivering weekly individual counseling to clients, as well as working with assignments in their Master Service Plan (MSP).

        Responsible for working with youth ages 17-24, which are reentering the community from an institution or another facility
        Gathering needed documents through JJIS (Juvenile Justice Information System) for Case Planning and creating an Initial Service Plan (ISP).
        Creating an Initial Service Plan for the Client based on a variety of factors, including behavioral, mental, and social health. Creating an environment for the youth to give back to the community in the form of volunteering and learning pro-social skills.
        Delivering weekly individual counseling to clients, as well as working with assignments in their Master Service Plan (MSP).
        Assisting with client community safety planning, activity planning and skill development, crisis intervention with up to 9 clients at a time
        Working with BRS (Behavioral Residential Services)
        Responsible for providing a range of crisis intervention, case management, and counseling services with an emphasis on solution focused outcomes to high-risk and troubled adolescents and their families.
        Monitoring and following through with clients regarding their case plans, in part through the delegation of tasks to Mentors. This usually includes crisis management while still covering the floor and attending to the safety of other clients as well as tracking community clients all at the same time.
        Working closely with community services staff to facilitate connection of adolescent and family.
        Working closely with the client’s Juvenile Court Counselor, and or Juvenile Parole/ Probation officer.
        Providing the Client, a positive road map to their future through monthly review meetings with the Clients Family, support system, counseling team and their Juvenile Court Counselor (JCC) OR Juvenile Parole/ Probation Officer (JPPO).
